What Is the Support at Home Program?
The Support at Home Program is the Government’s new model for aged care at home, designed to bring several existing aged care programs into one streamlined system.
It will replace:
1. Home Care Packages and Short-Term Restorative Care from 1 November 2025, and
2. The Commonwealth Home Support Program from 2027.
This new approach means less complexity, more choice, and improved access to high-quality support at home.

- Listen to your goals and design a plan that fits your lifestyle
- Coordinate clinical care, independence supports and everyday living services
- Guide you through agreements, pricing and statements in plain language
- Adjust your services as your needs change and help request reassessments when needed
What the Support at Home Program Offers
Under the new system, you can access a wide range of care at home supports, including:
- Clinical Care - Government-funded nursing, allied health and clinical services delivered safely at home.
- Independence Supports - Personal care, medication assistance, transport, respite and day-to-day help to maintain independence.
- Everyday Living Supports - Cleaning, gardening, shopping, home maintenance and other tasks that help you live comfortably at home.
You’ll receive a personalised budget based on your care needs, with eight funding tiers for greater flexibility. The program also includes short-term support pathways such as:
- Restorative care
- Assistive technology and home modifications
- End-of-life support
You only pay for the services you use, and contributions are based on your income and assets.
